Any Community Health & Wellness patient that has an urgent matter but does not require emergency attention can call 860-489-0931;  The answering service will put you in touch with the on call provider. Please be available for a return call.

Translation is available for patients with limited English proficiency.

If you have an emergency situation, please call 911 immediately or go to the nearest emergency room.
