Community Health & Wellness Center (CHWC) is an award-winning 501(c) (3) not-for-profit and the only comprehensive Federally Qualified Healthcare Center (FQHC) in the northwest corner of Connecticut.
 
We provide a full range of integrated healthcare services that includes medical, dental, chiropractic, nutrition, podiatry, and behavioral health as well as an in-house pharmacy – to over 7,000 patients who otherwise might not have access to healthcare.
