Any Community Health & Wellness patient that has an urgent matter but does not require emergency attention can call 860-489-0931;  The answering service will put you in touch with the on call provider. Please be available for a return call.

Translation is available for patients with limited English proficiency.

  • Drive approximately .1 mile

  • Turn right onto CT-8 North/Watertown Avenue ramp
  • Stay straight to go onto ramp
  • Merge onto CT-8 North
  • Stay on CT-8 North for approximately 19 miles
  • Take Exit 44 for US-202 toward CT-4/Downtown Torrington
  • Stay straight to go onto Christopher Road
  • Turn left onto East Elm Street/CT-4
  • Take first left (at light) onto East Elm Street
  • Drive approximately 1.2 miles on East Elm Street
