Community Health & Wellness Center (CHWC) is a nonprofit 501(c)(3) organization and the only comprehensive Federally Qualified Health Center (FQHC) in the Northwest Corner of Connecticut.

​We provide a full range of high-quality, integrated healthcare services, including medical, dental, chiropractic, nutrition, podiatry, and behavioral health, along with an in-house pharmacy. Our commitment enables us to serve over 7,000 patients who might otherwise lack access to essential healthcare.
