Dr. Ira Salom, MD, MSc (Med), FACP, CPE – Medical Director
Prior to joining Community Health & Wellness Center as Medical Director in 2017, Dr. Salom spent 6 years working as Chief Medical Officer for hospitals of the US Indian Health Service in both New Mexico and South Dakota. Dr. Salom graduated from the State University of New York at Buffalo, School of Medicine in 1977. Dr. Salom holds two Master’s Degrees in health policy and administration, and the second in nutrition. He is certified in both internal medicine and clinical pharmacology. Dr. Salom is the recipient of numerous awards including the US Army Commander’s Award for Public Service medal and the US Public Health Service reserve Physician of the Year. He is a fellow of the American College of Physicians, American College of Nutrition and New York Academy of Medicine.
Dr. Sally Knowles, MD
Dr. Knowles brings over 30 years of experience to Community Health & Wellness Center. She obtained a Bachelor of Science degree in Biophysics from the University of Connecticut and graduated from Southwestern Medical School in Dallas, Texas. After medical school, she completed her residency in internal medicine at Good Samaritan Hospital in Cincinnati, Ohio. Dr. Knowles is certified by the American Board of Internal Medicine.
Dr. Larry Deutsch, MD, MPH
Larry Deutsch, MD, MPH, is a pediatrician and a member of the Hartford City Council. He received his medical degree from the University of Wisconsin and after residency at the University of Connecticut he worked for years as a pediatrician in Boston and Hartford. Dr. Deutsch later received a Master’s in Public Health and directed a project to improve community health care with computerized medical records for children getting healthcare in community centers, school clinics, hospitals, and offices. His voluntary medical work has included membership on the Board to Trustees of Connecticut’s Tobacco and Health Trust Fund, Board of Directors of the Biomedical Engineering Alliance and Consortium, FEMA trips to New Orleans after Katrina and the Dominican Republic with Haitian refugees. He has three children and has been involved in PTO affairs as well as a school doctor. He continues work in pediatric care while also involved in public health issues such as education advocacy in the state legislature to establish a universal health care system which fairly and effectively benefits all residents.
Katrin Moskowitz, RN, APRN, DNP
Katrin (Kat) Moskowitz, APRN, DNP has over has over 10 years of experience in the profession of nursing. She was first licensed as a Registered Nurse in 2007, and then earned her BSN from Drexel University in 2011. She continued her education to earn both a MS in Nursing-Family Nurse Practitioner and her DNP from Frontier University. Kat was instrumental in starting our suboxone program here at CHWC.
Roseni Pease, RN, MSN, APRN
Rose is a dedicated Advanced Nurse Practitioner with many years of cardiology experience. She brings many years of healthcare experience to CHWC, having worked as a cardiology and pulmonary RN prior to becoming an APRN. Rose received both her BS in Nursing and her MS, Family Nurse Practitioner from Pace University. She is certified through the American Academy of Nurse Practitioners.
Dr. Waile Ramadan – MD – Physician
Dr. Ramadan joined Community Health & Wellness Center in 2017. He is a graduate of Baylor University with a Master of Science degree in Biology. He received his M.D. degree from American University in Beirut, Lebanon and completed his residency at Lutheran Medical Center in Brooklyn, New York.
Jaimie Machado, RN, MSN, APRN
Jaimie Machado, APRN earned a Master of Science degree from the University of Saint Joseph, as a Family Nurse Practitioner. Jaimie brings many years of healthcare experience to CHWC, having worked as a cardiothoracic and critical care RN prior to becoming a nurse practitioner.
Dr. James O’Halloran, DO – Physician
Dr. O’Halloran joined Community Health & Wellness Center in 2017. He is a graduate of the College of the Holy Cross, having received a Bachelor of Arts degree in 1979. He received his Doctor of Osteopathic Medicine (D.O) from the University of New England in 1985 and specializes in infectious disease medicine.
Mary Romano, MSN, APRN
Mary Romano, MSN, APRN, a lifelong resident of Litchfield County, has been an RN for 20 years. A graduate of University of St. Joseph with a Masters in Science, she is a family nurse practitioner. Previously, she worked in long-term care, following practicing school-based health care.
Caitlin Minihan, APRN
A native of Litchfield County, Caitlin graduated from the University of Saint Joseph with her APRN (FNP) in December of 2017, and joined CHWC in July 2018. She previously worked as an RN for nine years, specializing in cardiothoracic surgery.
