If you are a new patient who has an appointment at the center you can; click on New Patient Forms, print registration form, complete form and bring the completed form to your appointment along with your drivers license and insurance card if you have one.
If you are without health insurance coverage and have an appointment at the center there is financial assistance for those who qualify. You may; click on New Patient Forms, print a financial assistance application fill out the forms and bring required documentation to your appointment, our financial couselors will process your paperwork to see if you qualify for a sliding fee. All copays and payments are due at time of service.
