Nancy Wong, DMD, MPH- Dentist
Interim Dental Director
Dr. Wong joined Community Health & Wellness Center in 2017. Dr. Wong received her D.M.D and a Master’s Degree in Public Health from the University of Connecticut. She completed her dental residency program at Saint Francis Hospital and Medical Center in Hartford, serving as Chief Resident in her final year. Dr. Wong is an Urban Health Scholar, an enrichment program for collaborative care, medical management of vulnerable and underserved populations and public service. She has participated in many mission programs, both in Connecticut and abroad.
Tessie Jacob, DDS
Dr. Tessie Jacob received her Doctor of Dental Surgery degree from Howard University College of Dentistry in Washington, DC. She received her Bachelor of Science degree in Health Sciences from Stony Brook University in New York. She furthered her training in General Practice Residency at The Brooklyn Hospital Center located in Brooklyn, New York.
Dr. Jacob strongly believes in dental excellence and compassionate care. She also believes that education is an ongoing commitment, and keeps up to date with dentistry through continuing education lectures and courses. Dr. Jacob is a member of the American Dental Association and the American Association of Women Dentists. Dr. Jacob finds great happiness in helping people achieve confidence in their smiles and looks forward to making their experience in the dental clinic comfortable and positive.
Shalin Patel, DMD, MPH- Dentist
Dr. Patel was pleased to join CHWC in July 2018. He received his DMD from the University of Connecticut and completed his General Practice Residency at Danbury Hospital. He participated in the Urban Health Scholars Program (UHS) and is committed to providing the highest-quality dental care, especially to the underserved community. In his spare time, Dr. Patel enjoys cooking and playing tennis.
Josseelaine Diaz, R.D.H-Dental Hygienist
Jossee Diaz is our Registered Dental Hygienist at Community Health & Wellness Center. She received her Dental Hygiene degree in 2015 from Lincoln College of New England, where she was an honor student and recipient of the Liguori Merit Scholarship. Committed to helping her patients achieve excellent oral health, she puts an emphasis on preventative care. She is a regular volunteer at the “Mission of Mercy” annual events in both Hartford and Danbury, providing free dental cleanings to those in need. Jossee speaks fluent Spanish.
Shaleah Small, R.D.H-Dental Hygienist
Shaleah Small is a Registered Dental Hygienist at Community Health & Wellness Center. She received her Dental Hygiene degree from Lincoln College of New England back in 2015. Shaleah started her dental career as a dental assistant 16 years ago and has volunteered with “Give Kids a Smile” and “Mission of Mercy.” As a dental hygienist, Shaleah enjoys sharing her dental knowledge with all patients and giving personalized care that begins with talking about dental concerns and tailoring her care to your special needs.
