Heather Platt, APRN is providing primary health care. She is a graduate of Sacred Heart University with a Master’s Degree in Nursing and has over 15 years of clinical experience. Heather is Board Certified as an Adult Nurse Practitioner and a Diabetic Educator.

Mondays, Tuesday, Thursdays & Fridays
8:30AM to 5:00PM

Call our Torrington office at 860-489-0931 on Wednesday

We participate with most insurance plans.
Financial assistant with sliding fees are available for non-insured patients.
Fees will be based on the total number of household members and family income.
